The Value of Care Management Programs

One of the prominent techniques for improving health care delivery, particularly for chronic disease patients, has been care management. Two primary methods of care management are disease management and care coordination. Disease management currently has a checkered reputation, owing to a perception that it has failed to deliver promised cost savings. This Issue Brief examines the evidence that both of these forms of care management deliver value—do they improve health outcomes and what is their impact on costs.
